Teeth Whitening Sensitivity — What to Expect and How It Is Managed
The Short Answer
Teeth whitening sensitivity occurs when hydrogen peroxide temporarily penetrates the enamel and reaches the dentinal tubules — microscopic channels that transmit sensation to the tooth’s nerve. It is temporary, expected in some patients, and does not cause permanent damage. The Mechanism
Why teeth become sensitive during whitening
Teeth whitening sensitivity has a specific and well-understood mechanism. Professional whitening gels — including hydrogen peroxide at clinical concentrations — penetrate the enamel by diffusing through its porous surface structure. Once past the enamel, the gel reaches the dentinal tubules: microscopic channels that run from the outer tooth surface through to the pulp where the nerve lives. Temporary changes in fluid pressure and chemistry within these channels stimulate the nerve endings, producing the sensation most patients describe as brief sharp “zingers” or cold sensitivity. This is a transient response. The nerve is not damaged. The enamel is not permanently altered. As the whitening gel clears from the tubules and the enamel remineralises in the hours following treatment, the sensitivity pathway closes and sensation returns to baseline.
The degree of sensitivity varies considerably between patients. Enamel thickness, baseline dentinal permeability, the concentration and activation method of the whitening gel, and whether post-treatment desensitisation is applied all influence how much — if any — sensitivity a patient experiences. Many patients experience none at all.
Duration
How long does whitening sensitivity last?
At Dentalis, most patients find teeth whitening sensitivity resolves within 24 hours. This is due in part to the InnoSense post-treatment gel applied at the end of every in-chair session — addressing sensitivity through nerve depolarisation (Potassium Nitrate), enamel remineralisation (Sodium Fluoride), and physical tubule occlusion (nano-HA) simultaneously.
Without chairside desensitisation, whitening sensitivity typically resolves within one to three days following an in-chair session, and may persist for up to a week in patients with pre-existing dentin hypersensitivity or thin enamel. Individual response varies — this is an expected clinical range, not a sign that something has gone wrong.

Potassium Nitrate (KNO₃)
Depolarises the nerve endings within the dentinal tubules, blocking the electrochemical signal that transmits sensitivity sensation to the pulp. This is the same active ingredient recommended in desensitising toothpastes — applied here at the point of maximum post-treatment receptivity, directly on the tooth surface.
Sodium Fluoride (NaF)
Supports enamel remineralisation in the post-treatment window. The whitened enamel surface is transiently more receptive to mineral uptake. Fluoride stabilises the enamel crystal structure and reduces its permeability in the hours following whitening.
Nano-Hydroxyapatite (nano-HA)
Physically deposits into the dentinal tubules, occluding the channels that transmit sensitivity signals. Unlike nerve-blocking agents which manage the sensation, nano-HA addresses the physical pathway — plugging the tubule rather than quieting the nerve. Published systematic review and meta-analysis evidence supports a 39.5% reduction in dentin hypersensitivity with hydroxyapatite-based interventions compared to placebo.2

Risk Factors
Who is more likely to experience sensitivity after whitening?
Several factors increase the likelihood of noticing teeth whitening sensitivity during or after whitening. None are absolute contraindications — they are clinical variables assessed by Dr Jonathan Liu before every whitening session.
Pre-Existing Dentin Hypersensitivity
Patients who already experience sensitivity to cold drinks or sweet foods begin with a lower threshold before whitening begins. Pre-treatment use of a KNO₃ or nano-HA desensitising toothpaste for two weeks before the session can reduce this baseline sensitivity measurably.
Thin or Worn Enamel
As enamel thins with age or acidic erosion, the dentinal tubules are closer to the surface. Older patients and those with bruxism or a history of acidic diet are more likely to notice sensitivity during whitening — the pathway from gel to nerve is simply shorter.
Gum Recession and Exposed Root Surfaces
Root surfaces are covered by cementum rather than enamel — a less protective barrier. Where recession has exposed the root, the whitening gel reaches the nerve via a shorter pathway. InnoDam gingival protector is applied to all exposed tissue at Dentalis before every session. A professional clean beforehand also supports optimal gel contact on clean enamel.

When to Seek Care
When sensitivity after whitening is not normal
Brief zingers and temperature sensitivity in the 24 hours following are expected forms of teeth whitening sensitivity. The following signs fall outside the normal pattern and should be reviewed by a dentist.
Sensitivity persisting beyond one week with no sign of gradual improvement after the whitening session ends.
Sharp, throbbing, or unprovoked pain not triggered by temperature or food. This pattern is more consistent with pulpal pathology than whitening-induced sensitivity.
Sensitivity confined to one tooth — particularly one with a history of trauma, prior root canal treatment, or a large restoration. Localised persistent pain can indicate pre-existing pathology that whitening has unmasked.
Swollen, inflamed, or bleeding gums that do not settle within 24 hours. Contact with unprotected soft tissue can cause chemical irritation that warrants clinical assessment.
